Everything you need to get your child started at USTC Branch.
How to Enroll
Enrolling your child at USTC Branch is simple. Download our enrollment form, fill it out, and bring it with you when you visit — or send it to us by email.
Download the Form
Our enrollment packet includes the USTC enrollment form, medical history record, and required state forms.
Fill It Out
Complete all sections including child info, parent/guardian contacts, emergency contacts, and medical history. You'll also need an immunization record and physical exam.
Submit & Visit
Bring the completed form to our location at 650 Branch Avenue, or email it to [email protected]. We'll get your child set up and ready to start.
Student Enrollment Form
PDF — includes enrollment form, medical history, CCAP enrollment agreement, and RI DHS child information form.
CCAP Families
If your family is enrolled in Rhode Island's Child Care Assistance Program (CCAP), the enrollment packet includes the Parent/Provider Enrollment Agreement. Our Provider ID is 178 603 (MK Taekwondo Inc DBA USTC Branch).
